This is to notify to the students of 3^rd & 4^th Batches BTHM program that the date of your Form Fill-up has been ascertained according to the following schedule:

The students are advised to collect their Forms from the office after clearing their dues and to bring the last semester admit card, registration card (Photocopy) and 2 copies of pp size photographs for form fill up.
